WHAT IS IOP?
IOP provides evidence-based behavioral health care for those who need more support than outpatient care but do not need inpatient hospitalization.
Programs are clinician-led and deliver meaningful group-based therapy multiple days a week. Patients are stabilized while building skills and staying connected to their community.
Our Approach
Patients enter the program through health center referrals or proactive, data-informed identification.
Structured clinical screening is used to determine whether IOP is the appropriate level of care, ensuring patients are matched to treatment based on clinical need. Care delivery is integrated with health center teams and supported by onsite clinical liaisons to maintain continuity throughout the transition into programming and beyond.
In practice, this means:
- Health center teams refer patients directly for clinical evaluation
- Patients with higher-acuity behavioral health needs are proactively identified using defined clinical criteria
- Onsite clinical liaisons support warm handoffs from health center providers into programming
- Intake screening and IOP appropriateness are completed in partnership with health center teams
- Ongoing patient care coordination ensures follow-up and closes gaps in care
STEP 1
Intake
Once eligibility is confirmed, our intake focuses on understanding the patient’s needs, goals and readiness for care.
- Intake appointments assess symptom severity, treatment goals and program fit
- Structured programming is scheduled based on individual need, with flexible session timing to support consistent participation
- Clear expectations are established to prepare patients to actively engage in programming
Patients receive the level of care they need without stepping away from work, family or community.
STEP 2
Group and Individual Therapy
ComoCare designs and delivers customized care programs to reduce relapse and strengthen coping skills. We work alongside clinic staff to deliver higher-acuity care without disrupting established workflows.
